Issue 592: 3.14: TV-out missing

OMAP-DSS has chaged significantly from 3.14 to 3.15
So we have to rework and re-integrate our OPA362 video amplifier 
driver and add device tree bindings for it.

Status as of 3.18-rc3:

tvout is now available, but there are still problems:
* the frame buffers are assigned as fb0: tvout, fb1: lcd
* but if trout is disconfigured, fb0: lcd
* this breaks a static xorg.conf
* switching by the ./tvout 0/1 script works, but uses the tv 
resolution (landscape) for the LCD
* after booting, the default display (LCD) is NOT enabled and we 
have neither trout nor LCD

All this appears to come from a race within deferred probing which 
mixes up the omapfb driver.

Still, the core changes for venc, device tree and opa362 can be (and 
have been) submitted upstream.
