Issue 438: <form> missing/incomplete features

* <fieldset> is not tested
* <select size=n> does not show a table (optionally with 
multiple selections)
* what does a table with multiple selections send?
* <input type="file"> should track the mouse when 
the button is pressed and pop-up only if released inside
* how is a file upload sent (encoded) as GET or POST request?
* <textarea> is not working (editable NSTextView inside 
NSViewAttachmentCell)

and a reset button resets all fields to their initial value!
<textarea>:   text provided
<input>:      value provided
<input file>: no file selected
<select>:     initial selection(s)
<input box>:  initial radio button/checkbox state

maybe, we should remove the value-initialization (default values) 
from the structure initialization (view and cell hierarchy) to 
_reset: and call _reset when the <form> is initially loaded.

Some features are missing in work. So if you want to start them
